Poznana sta dva mehanizma, preko katerih lahko sklepamo na pove\u010dane energetske potrebe pri obolelih z rakom.<\/p>\n<p>Eden izvira neposredno iz signalnih poti tumorskih celic, drugi pa je nasledek odgovora organizma na stres.<\/p>\n<p>Posledica obeh mehanizmov so presnovne spremembe, ki povzro\u010dijo premike v presnovnih poteh k razgradnji mi\u0161ic in zmanj\u0161ani izgradnji le-te.<!--more--><\/p>\n<p>Pomembno je razumeti, da moramo organizem, v katerem so se namno\u017eile rakaste celice presnovno podpreti, saj mu s tem omogo\u010damo sprotno regeneracijo, bolj\u0161i odziv na specifi\u010dno onkolo\u0161ko zdravljenje in s tem pove\u010dujemo mo\u017enost ozdravitve.<\/p>\n<p>Ne glede na koli\u010dino vnosa hranil, si bodo nediferencirane rakaste celice vzele svoj dele\u017e za pokritje energetskih potreb.<\/p>\n<p>Ob stradanju, oziroma postenju, bomo na\u0161e telo \u0161e bolj podvrgli propadu, saj primitivne rakaste celice ne poznajo adaptacijskih regulatornih mehanizmov ob pomanjkanju hranil in si \u00bbvzamejo\u00ab svoje kar na ra\u010dun gostiteljevega telesa.<\/p>\n<p>V sodobni medicini so ob iskanju \u010dude\u017enega zdravila za ozdravitev raka kaj kmalu odkrili izrazit pomen potrebe po vzporednosti obeh terapevtskih poti &#8211; tako specifi\u010dnega onkolo\u0161kega zdravljenja raka kot presnovne podpore.<\/p>\n<p>Ve\u010dkrat je bilo dokazano, da je uspe\u0161nost onkolo\u0161kega zdravljenja bolj\u0161a, \u010de je bolnik ob tem dobro prehranjen in po mo\u017enosti \u010dimbolj telesno aktiven.<\/p>\n<p>Obi\u010dajno so ob prihodu v bolni\u0161nico na diagnosti\u010dne postopke pri bolnikih z rakom \u017ee izra\u017eeni znaki telesnega propadanja kot posledica raka in zraven \u0161e morebitnih prehransko\/stradalnih podvigov v poskusu samozdravljenja.<\/p>\n<p>S prehranskim pregledom ugotovimo, kako napredovalo stanje telesnega propada je prisotno in se odlo\u010dimo za ukrepe. Najpogostej\u0161i ukrep, ki hkrati tudi odra\u017ea zadovoljivo stanje bolnika, je prehransko svetovanje klini\u010dnega dietetika.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter size-full wp-image-2623\" alt=\"proteini\" src=\"https:\/\/www.zdravstvena.info\/preventiva\/wp-content\/uploads\/2013\/12\/proteini.jpg\" width=\"272\" height=\"185\" srcset=\"https:\/\/www.zdravstvena.info\/preventiva\/wp-content\/uploads\/2013\/12\/proteini.jpg 272w, https:\/\/www.zdravstvena.info\/preventiva\/wp-content\/uploads\/2013\/12\/proteini-150x102.jpg 150w, https:\/\/www.zdravstvena.info\/preventiva\/wp-content\/uploads\/2013\/12\/proteini-220x150.jpg 220w\" sizes=\"auto, (max-width: 272px) 100vw, 272px\" \/><\/p>\n<p>Na ta na\u010din dobijo bolniki osnovna navodila za kritje potreb po beljakovinah in energiji ter potrebi po telesni aktivnosti. Ob odhodu iz ambulante, kjer prejmejo tovrstno svetovanje, pa so vseeno prepu\u0161\u010deni svoji domi\u0161ljiji in zmo\u017enostim za sledenje tem navodilom.<\/p>\n<p>Ob oceni, da je telesno propadanje preve\u010d izrazito ter da je tudi specifi\u010dno onkolo\u0161ko zdravljenje pustilo svoj pe\u010dat, prehransko svetovanje ni zadostno.<\/p>\n<p>V tem primeru poskusimo poleg osnovnega svetovanja \u0161e z uvedbo posebnih hranil, za katere vemo, da imajo poleg hranilnega tudi zdravilni u\u010dinek. Najve\u010dkrat so to omega 3 ma\u0161\u010dobne kisline, specifi\u010dne aminokisline, mikrohranila in vitamini (predvsem D).<\/p>\n<p>Kadar tovrstni ukrepi niso zadostni oziroma da ima bolnik subjektivne te\u017eave zaradi osnovne bolezni ali specifi\u010dnega zdravljenja, najve\u010dkrat v obliki drisk, bruhanja, neje\u0161\u010dosti, infektov, je potreben sprejem na oddelek in dodatno nadome\u0161\u010danje hranil v \u017eilo.<\/p>\n<p>Do nedavnega je veljalo prepri\u010danje, da mora bolan \u010dlovek po\u010divati, mirovati, celo le\u017eati.<\/p>\n<p>Vemo, da v akutni fazi obolenja ponavadi res nismo zmo\u017eni pretirane telesne aktivnosti. Pomembno je, da na primer bolniki \u017ee v bolni\u0161nici pri\u010dnejo s fizioterapijo in nato doma nadaljujejo z redno telesno aktivnostjo v obliki uporovne vadbe.<\/p>\n<p>Dokazano je, da uporovna vadba upo\u010dasni, ob primernem vnosu beljakovin lahko celo zavre s starostjo povezano propadanje mi\u0161i\u010dne mase. Pri onkolo\u0161kih bolnikih pa vsaj izbolj\u0161a izkoristek zau\u017eitih hranil kot gradnikov v obnavljalnih procesih.<\/p>\n<p>Zagotovo je v \u017eivljenju posameznika eden najte\u017ejih trenutkov prav tisti ob postavitvi diagnoze raka. Soo\u010denje s tem in nato sprejemanje specifi\u010dnega zdravljenja z vsemi posledicami zahteva veliko energije tako prizadetega bolnika kot njegovih bli\u017enjih.<\/p>\n<p>Po principu vzporednega zdravljenja s specifi\u010dno onkolo\u0161ko terapijo in presnovne podpore je optimalno \u017ee pred uvedbo zdravljenja pri\u010deti z zadostnim vnosom hranil.<\/p>\n<p>V tem trenutku se glede potrebne dnevne koli\u010dine beljakovin in energije dr\u017eimo priporo\u010dil za prehrano onkolo\u0161kega bolnika.<\/p>\n<p>Ob tem ne smemo pozabiti na redno telesno aktivnost.
